Be surprised, but every site is different, so what works perfect for one design / structure / content, might not work at all for someone else's website. all depends weather the ads blend with the design or stand out.
if u r using adsense, i would recommend setting channels for every ad, or place of ads, i.e. upper_navigation, site_navigation, etc, so that u will get a general idea what channel brings more revenue. if, for example, u see that after a week channel 1 brought 20$, and channel 2 - 1$, u would want to remove the ads from channel 2 fully, in order to increase the click's bid of the first channel on the page, which gets more clicks. remember that the less ads u have displayed -- the higher is the bid per click.
Do place adsense:
at the upper navigation
left side navigation
the upper / middle part of the page
Do NOT place adsense:
at the very bottom
on the lower part of right navigation
above the header
from personal experience i would say that the ads get clicked the most when the visitor thinks that the ads is just a link inside the page, or a navigation link. when i (and again, its maybe only me) place a big image ad at the top of the site -- it got no clicks at all. maybe because people knew its an advertisement, and didnt want to click it, but rather preferred to browse the content of the site itself.
this also means that best way to display adsense is without borders, and make it merge with the design.
also, its important to play with the placement from time to time, because regular visitors will soon learn where the ads are, and ignore them, so i'd suggest to modify the colours scheme / ads location occasionally.
